Dr David Chandy: Testosterone therapy risks and benefits

Testosterone therapy risks and benefits: The US Army has announced a new policy requiring all male service members aged 30 and older to undergo annual screening for testosterone deficiency. Defence Secretary Pete Hegseth stated the program aims to ensure troops maintain optimal hormone levels for peak performance. Those found with low testosterone may receive testosterone replacement therapy (TRT), but experts warn about potential complications from routine screenings and treatment.

Dr David Chandy, director of endocrinology at Sir HN Reliance Foundation Hospital in Mumbai, explained that testosterone levels naturally decline by one to two percent each year after age 30. By their forties and fifties, many men develop onset hypogonadism, experiencing fatigue, reduced libido, mood changes, and muscle loss. However, these symptoms can stem from other causes, so proper testing is essential before diagnosing low testosterone.

Testosterone therapy offers benefits such as improved energy, sexual desire, and quality of life for men with clinically low levels. About one in ten US men have diagnosable low testosterone. Yet Dr Chandy highlighted significant risks, including increased red blood cell production, worsened sleep apnea, reduced fertility, and prostate issues. Women also produce testosterone, which supports sex drive, muscle strength, and bone health, but levels drop after menopause.

The important takeaway is that testosterone deficiency is treatable, but self-diagnosis is dangerous. Individuals should consult a hormone specialist and undergo proper testing before considering therapy.
